  1. I was challenged by a biochemist on the issue of hypothyroidism causing hyponatraemia. They stated this simply isn’t true, save for patients in extremis in a myxoedemic coma. Is there good evidence that hypothyroidism causes hyponatraemia. How common is it?
    Or is it just a theoretical concern which we don’t see in clinical practice.?

    1. In short, no there is no good evidence, and hyponatremia likely exists in an association with myxedema.

      It appears that outside of myxedema/decompensated hypothyroid, there is virtually no difference in the incidence of hyponatremia among euthyroid and hypothyroid patients (https://www.ncbi.nlm.nih.gov/pubmed/9365479/). But as your colleague stated, there appears to be at least an association in the extreme spectrum of hypothyroidism. Based on a limited evidence, in an infrequent clinical scenario, it is difficult to assign an incidence.
